Credit Card Authorization

Instructions

The completion of this process is required in order to make a University purchase using a University credit card.

What do you need to complete this process?

• Approver details – name, email address. This person must be an approved signer for the project # you specify.
• Details of purchase: vendor, amount, description.
• Please note that ONE submission per vendor is required.

Once this form is completed, the submission will be emailed to the approver(s) for digital signature.

Transactions with Staff

Any transactions with another staff member must be pre-approved by the Finance Working Group (FWG). This includes transactions with vendors like Turo and AirBnB. To contact the FWG, please email [email protected].

You have specifid that the purchase is to be charged to a restricted account. This requires 2 approvers. Please specify below the 2 names and email addresses of the approvers.

Purchase Details

By clicking the Submit button below, your request will be sent to the approver(s) you specified.

Because the purchase amount is greater than $2,000.00, this request will also require approval from the University CFO team.

Please email the submission to either Walter Fenner, Paul Randerson or Pieter Heres.

Once the request is digitally signed, you will receive the signed and completed request.
