New Missions Account

We'd like to know a little bit more about your missionary service now, and your anticipations for the next 2 years.

As you've mentioned your spouse serves at U of N, Kona on a part-time, or full-time basis, we'd like to know the same information for them.

We'll need to know your contact info, such as mailing address, email and contact number.
We will use the email address below to inform you of upcoming disbursements, as well as general Mission Account communications.

What are you collecting?
We will need to know who you are, and whether or not you're eligible to receive funds from our organization. This is similar to an "I-9 verification" that most employers do.
 
Work Authorization
You must provide evidence of your ability to work/receive funds in the United States. Examples may include a U.S. Passport, a DHS issued Employment Authorization Document (Form I-766), Lawful Permanent Resident Card (Form I-551), or other evidence of DHS issued work authorization, e.g. R-1 visa with Form I-94.
 
Social Security Number
Because we issue a Form 1099-NEC, we need your Social Security Number. For this, you can present a copy of your Social Security Card, a previously issued Form 1099 or W-2 with your name on it, or an old Federal or State tax return.
 
When your account is approved, you will need to bring these evidences to the Mission Account training/setup meeting.

On an approved Mission Account, providing that you remain eligible, we disburse funds every 15th and end of the month (if it falls on a weekend or holiday, then the business day prior). This is usually via direct deposit..
 
If your account is approved, please bring your Routing # and Account # OR a voided check to the Mission Account training/setup meeting. This will be needed to set up Direct Deposit.

We are required to verify this request. In addition to our Staff Services office, weʻd like to contact your leader to verify your answers.
 
This must be a SENIOR leader at the YWAM UofN Kona/U of N, Kona campus. Examples include a Department Leader, a Ministry Leader, or a Circle Leader.
 
For this purpose, we do not accept one-off school leaders, or leaders outside of the YWAM UofN Kona/U of N, Kona campus.
